Reset Client Polaris Credentials

Clear a partial Polaris credential state and mint a fresh principal.

For recovering a client stuck with some-but-not-all of the three credential columns set — the normal mint refuses to operate on a partial row. Clears the orphaned columns and mints via the client's key. A fully provisioned client is returned unchanged.
