Update effective date on a group-level practitioner specialty

Updates the effective start date on a practitioner's specialty association at the group level. Use this when the effective date on an existing group-level specialty record needs correction; this is the broadest specialty level, so for location or network specificity use the corresponding group-practitioner-location-specialties or group-practitioner-location-network-specialties endpoints instead. The target specialty association must already exist.
