List locations associated with a practitioner

Returns all locations the specified practitioner is associated with. Use this to retrieve a practitioner's location memberships; for location data with full relationship graph context, use GET /practitioners/{id}/relationships/locations instead. {practitionerId} is the certifyPractitionerId. Terminated associations are included; this endpoint does not filter by termination status.
