Remove a hospital affiliation from a practitioner

Permanently removes the specified hospital affiliation from the practitioner record. Use this when a hospital affiliation is no longer valid and should be removed; this is a hard delete, and the affiliation cannot be recovered after deletion. {practitionerId} is the certifyPractitionerId, and {affiliationId} is the UUID assigned by the server when the affiliation was created, returned in the POST response. The hospital affiliations feature must be enabled for the tenant.
