Reauthorize Mcp Oauth

Apply a completed pending OAuth flow to an existing MCP integration.

Merges the pending row's tokens onto the existing integration's secret, rotating credentials in place.

Permissions: integration setup user — matches the permission required to create the integration in the first place.
