Distributor View Create Team Users

Seat brand new users on the distributor's own client, each with a distributor role.

The grant endpoint above can only reach users who already exist, which is a dead end for a distributor whose every user already holds a role. This one provisions the accounts.

Every row is checked before anything is created: neither Okta nor the users table rolls back, so a bad role or an address already in use has to fail the batch while it is still cost-free to fail.
