Create a questionnaire template

Creates an empty draft template for the calling company. Unlike creating a questionnaire, nothing is cloned and the name is not checked for uniqueness — a company may hold several templates under one name. A Retraced admin's template is Retraced-owned and starts inactive, so it stays hidden until it is switched on.
