Submit a Termly Health Form

Requires permissions: Guardian or Parent

Submits a health form for one of the parent's children. The form is open all year: parents may submit whenever something changes, not only in a termly window. A submission that reports an actual change emails the pupil's housemaster and the Medical Centre.
