Get holdings by chain and custody source

Returns the caller-group's current settled holdings bucketed by (chain, source) → asset. Chain is attributed per money account (account type + custodial coin), never from ledger payment_rail; the custodial Go Network omnibus is reported as off_chain and an unresolved chain as unknown.
