Refine Deal
Refresh strategy + draft for a single deal using new context.
The user types into the dialog's "Refine plan" card; this enqueues a pipeline_deal_replan job carrying the user_context and a fresh pipeline_run_token. Older queued replans for this deal abort as superseded once they pick up. Does not queue a cross-deal learning signal — this input is a per-deal refinement only.
