List Pending Permission Requests Route
Approvals waiting on this person that have no chat thread to live in.
Today that means MCP-origin requests (ChatGPT, Claude connector, Cursor, the Every CLI). The destructive-write gate creates a row and waits; the push notification announces it; this is where a surface can actually FIND it. Without this endpoint the gate could only ever block, never be satisfied.
Chat-origin approvals are deliberately excluded -- they already appear inline in their own thread.
