`GET /api/consent_forms`

Retrieve or search patient consent forms

## Query parameters

- `cursor` string — The pagination cursor value.
- `page_size` integer — Number of results to return per page.
- `doctor` integer

## Response `200`

OK

- object — Paginated Result
  - `previous` string — Previous paginated page
  - `data` ConsentForm[] — result data
    - `archived` boolean
    - `is_mandatory` boolean, required — If true, consent form must be signed prior to appointment check in
    - `uri` string, required — Files are passed using `multipart/form-data` encoding, but returned as URLs.
    - `doctor` integer, required
    - `updated_at` string
    - `created_at` string
    - `title` string, required
    - `assign_by_default` boolean, required — If true, consent form will always be automatically assigned to appointments
    - `order` integer — The order of consent forms that will show in management screen
    - `id` integer
  - `next` string — Next Paginated page
