PATCH /clinic-config/inventory - Update inventory settings

When true, a dispense made outside an appointment context finds or creates a draft estimate for the patient today and attaches the line item automatically, so dispenses never float unbilled. Existing tenants default to false via the migration; new tenants opt in through the clinic settings UI.
